for the forgetful part write everything down. Make a daily planner for yourself and set up your phone with reminders. Plan and prioritize your daily activities and if unable to balance multi-tasking then don't do it. For the lazy part that's more difficult. If your body gets too much or too little sleep you will feel it thru out the day. Maybe adjust how you sleep and set a time to prepare for sleep. I'm currently inbetween jobs and still have a schedule, I wake up around the same time and go to bed around the same time. Humans need a steady routine or we feel out of place. Hope this helps.

I have taken a look at your channel and found some things you could improve.

To begin with your upload schedule, in your about section you mention 3 times a week but when looking at your video's i don't see that schedule coming back. Besides that use your about section to tell something about your channel, what makes you unique?

Second, your video description is on the short side, use this to tell some information about your video's. Note that this is also a good thing to use in your playlists. Use your playlist description to tell something about the content of that playlist.

Third, not all your video's have custom thumbnails so i would adjust that a bit.

Getting into a routine is one of the ways that has helped me, once you settle into it (which may take a few weeks) you may find it easier to maintain that consistency.

A good thumbnail is just as important as having good content in the video, they are not mutually exclusive and both play a part in not only getting you more views but also longer watch time which is the key to getting your videos ranked higher on YouTube. Usually, if your video has something interesting or exciting then you can use a screenshot of that and superimpose text on the bottom with information such as video title, episode number, etc.
